Heart failure is a common diagnosis in home care patients. Many of the clinicians are new to home care.  This education was developed for nurses, occupational therapists and physical therapists to promote excellence in heart failure management outcomes. 

Criteria for Successful Completion of Activity to Claim CE Certificate
  • Review of material (Chapters 1-5)
  • Each Podcast must be viewed to 100% before a completion will be granted by the system
  • Post-test with a minimum passing score of 80%
  • Evaluation completion with electronic signature

Introductory: Information geared to clinicians with little or no knowledge of the subject. Focus is on providing general introductory information.



Intermediate: Information geared to clinicians with a general working knowledge of current practice trends and literature related to the subject. Focus is on increasing understanding and competent application of the subject.

Learning Objectives

The learner will be able to:

Chapter 1

  • describe the classic pathophysiologic steps in the development of heart failure
  • recognize heart failure signs and symptoms
  • identify comorbidities and their impact on heart failure 

Chapter 2

  • identify AHA guidelines, and application of heart failure classification
  • state the principles of effective cardiac device therapy management
  • describe effective medication reconciliation for heart failure patients.

Chapter 3

  • explain health coaching concepts
  • identify key interventions that are part of the plan of care for a heart failure patient
  • recognize fundamental principles of exercise prescription for a heart failure patient

Chapter 4

  • state AHA guidelines for application of therapies for end stage heart failure and acute decompensated heart failure
  • identify end stage planning in effective management of heart failure patients.

Chapter 5

  • recognize importance of shared decision making
  • describe quality assurance and performance improvement in the management of heart failure

    Michele Berman, PT, DPT MHA

    Michele is a PT and has served as director of rehabilitation for the Bayada Home Health Care. Michele has worked in the home health setting since 1993 in direct client care, managerial and educational capacities whereby heart failure has been a focused area of interest in the home care setting.

    Allison Elliott, RN, MSN

    Allison is a Registered Nurse with over 37 years of healthcare experience, 22 years of those in home health care. She received her Bachelor of Science in Nursing from the University of Florida and a Masters of Nursing from Emory University. Her clinical background includes critical and cardiovascular care, medical surgical care and diabetes education. She has extensive experience in home health clinical operations, regulatory compliance and quality assurance and performance improvement activities to improve services, outcomes or costs.

    Mandeep K. Mangat, MD, MPH

    Mandeep is a physician and public health professional. She is the design lead for Population Health Management and Clinical Initiatives. She has over ten years of extensive clinical and administrative experience in hospital medicine, clinical operations, population health and operational efficiency improvement in both national and international settings. Her interests include outcomes management, results driven continuous improvement and multilevel determinants of population health. She received her medical degree (MD) from Saint Petersburg Medical Academy, Masters degree in Public Health (MPH) degree with a focus in Healthcare Administration from West Chester University and Lean Six Sigma Black belt (LSSBB) from Villanova University. She is an active member of American College of Healthcare Executives (ACHE), Lean Healthcare Exchange and Institute for Healthcare Improvement (IHI).

    Allison Sterner, RN

    Allison has 23 years experience in the nursing profession; including Emergency Room nursing, Home Health Nursing, and Community and Staff education in multiple disease management.  She served on the Heart Failure work group to collaborate on BAYADA staff education as it relates to Heart Failure and also to design an internal certification in Heart Failure management. She served as a member of the Nursing and Clinical Advisory groups to review best practice guidelines for Heart Failure and other disease management.

    Sharon D. Vogel, AB, MBA

    Sharon has her MBA in Healthcare Management and Strategy.  Sharon was a Director of Hospice Services at BAYADA Home Health Care.  She has presented on the basics of palliative care and Hospice 101 to various audiences.
